We support parents, carers and young people through the Education Health and Care plan process.

The Children and Families Act 2014 has changed the way children and young people from birth up to the age of 25 with special educational needs (SEN) and disabilities receive support.

As part of these reforms statements have been replaced by Education Health Care Plans (EHC).

An EHC plan brings together a child or young person's education, health and social care needs into a single legal document.

The purpose of an EHC plan is to make SEN provision to secure the best possible outcomes across education, health and social care and when moving into adulthood.

We offer free, impartial and confidential support to parents, carers and young people through the Education Health and Care plan process.

Our Independent Supporter, Sue Gough has completed accredited training by the Council for Disabled Children and receives regular supervision to carry out her role.

Sue works flexible with you and can be accessed at any stage of the Education Health and Care Assessment and planning process.

Referrals
This is a self referral process for parents, carers and young people.

We only accept referrals from professionals with your consent.
